Thank you for your interest in placing your child(ren) with me - this form briefly outlines my hours and fees and seeks more information from you in order to in order to see if I can offer you a place in my setting.
My core working hours are Tuesday - Friday 8am - 5pm term time only (Gloucestershire schools term dates can be found here) & I will try and accommodate your needs inside these hours if I can (subject to places being available)
I offer a minimum daycare session of 6 hours (9am-3pm) at £45
If you require additional hours outside of these sessions they will be charged at £8 per full hour
I accept the Government funding (check what you may be entitled to at https://www.childcarechoices.gov.uk) and also offer the tax free childcare scheme.
My fees outside of funded hours will be £8 per hour.

Please note I will offer places in order of whose requirements fit best into a place I have available, and also give consideration to the order in which people contacted me.
This form does not formally commit to a place, if I don't currently have space then I will hold these details on a waiting list and will get back in touch should a place become available, if at any time you would like to be removed please contact me.
